Reimbursement Request Form for Extensions

This is the Reimbursement Request Form for Spring 2025 or Fall 2024 Lawns to Legumes grant recipients with extensions. Fall 2025 grant recipients fill out their form on Submittable (see the Grantee Guide for details).

The deadline to submit a reimbursement request for grantees with Fall 2024 or Spring 2025 extensions was Sunday, November 30, 2025, at 11:59 pm Central. To request an extension through Spring 2026, email the L2L Helpline.

Fill out this form once you have completed your pollinator habitat project. Review required materials on the Grantee Guide and the Eligible Expenses Guide. Project Purchases

Please upload one receipt at a time, detailing your eligible expenses. Receipts must be legible and show the retailer name and date of purchase. Eligible expenses include: native plants, seeds, wood mulch, delivery fees, and contractor installation and/or maintenance. For the full list of eligible expenses, visit the Grantee Guide page. We cannot reimburse you if you do not share your receipts. If your receipt does not show the names of the plants/materials, please also upload a list of the plant/item names or a photo of the plant tags in the next section of this form.

How many receipts detailing eligible expenses do you have?

You only need to upload receipts for project expenses you are requesting reimbursement for. There is no need to document any eligible expenses beyond that.
